چکیده

Introduction Pulmonary vein isolation (PVI) is the standard treatment option in interventional electrophysiological therapy of atrial fibrillation. Three-dimensional (3D) mapping systems are commonly employed to guide PVI procedures. This report demonstrates how 3D mapping systems may be applied beyond their standard use to facilitate transseptal puncture and PVI in a case of challenging cardiac anatomy.

A surgical case of combined valvular disease complicated by absent right superior vena cava and persistent left superior vena cava.

A 74-year-old man with combined valvular disease with a recent cerebral infarction was admitted. While undergoing thorough examination for valvular disease, absent right superior vena cava (RSVC) and persistent left superior vena cava (PLSVC) were recognized. Chest X-ray film suggested a right arch protrusion, and CT and venogram confirmed the diagnosis. During surgery, replacement of the mitra...

Double transseptal puncture guided by real-time three-dimensional transoesophageal echocardiography during atrial fibrillation ablation.

A 65-year-old man was admitted to our centre because of recurrent drug-resistant episodes of paroxysmal atrial fibrillation, for pulmonary vein radiofrequency catheter ablation. Double transseptal (TS) puncture was performed under realtime three-dimensional transoesophageal echocardiography (RT 3D TEE) guidance. We started by placing a quadripolar catheter in the coronary sinus and a Pigtail ca...

Mediastinal Neuroendocrine Carcinoma of Unknown Origin Presenting with Superior Vena Cava Syndrome (SVCS): A Case Report

Primary neuroendocrine carcinoma (NEC) of the mediastinum is a rare type of carcinoma. According to the literature, only five cases of this condition have been reported so far. In this paper, we present a rare case of mediastinal NEC of unknown primary site. The patient was a 34-year-old man with mediastinal NEC, who presented with chronic dry cough and a right-sided mediastinal mass one year p...

Non-Fluoroscopic Transseptal Catheterization During Electrophysiology Procedures using a Remote Magnetic Navigation System.

Transseptal punctures are commonly performed, and left atrial (LA) access is frequently lost during lengthy, complex electrophysiology (EP) procedures. We describe a new technique for non-fluoroscopic re-crossing the fossa ovalis using a new multielectrode transseptal sheath (TS) and a new remote magnetic catheter navigation system (RMNS) (CGCI System, Magnetecs) that uses 8 rapid external elec...

P wave morphology of an arrhythmogenic focus in patients with atrial fibrillation originating from a pulmonary vein or the superior vena cava.

BACKGROUND It was hypothesized that atrial premature contractions (APCs) originating in the pulmonary veins (PVs) or superior vena cava (SVC) can be localized by evaluating characteristics of the P wave. METHODS AND RESULTS Thirty-eight patients with paroxysmal atrial fibrillation were studied. P wave polarity and morphology of the ECGs during pacing from PVs were analyzed and compared to tho...
